We ask that our new patients bring any relevant insurance information as well as a completed heath history form to the first visit. You can complete the health history form here at our website and print it out or submit it on-line, whichever is most convenient for you.
The initial examination is a fact finding mission. At this appointment we will introduce you to our staff, provide you with a tour of our office and introduce you to our doctors. We will then gather the necessary personal information and preliminary clinical information required for us to make a clinical recommendation. We will then take what is referred to as study records.
The study records are needed prior to treatment in order to prepare a case analysis and a comprehensive treatment plan. These records consist of impressions for models, photographs, and x-rays of your teeth.
We will provide you with a computer simulation of the treatment options available to you. During this case presentation, we will be discussing all future treatment objectives and the orthodontic methods by which these goals will be accomplished. Ideally, both the patient and parent (preferably both parents) of our younger patients should be present at this time, since the responsibilities of all parties involved will be discussed.
Our patients’ physical and emotional comfort is a priority to us. We always make sure to listen to our patients so that we clearly understand his or her needs. We explain each and every procedure in its entirety before beginning treatment, and we always give our patients the opportunity to ask questions.
